Required for All Travelers
Everyone traveling to, from, or transiting through American Samoa must complete an ASDAC Traveler Declaration, including U.S. Citizens, U.S. Nationals, and residents.

Submission Window

One Form Per Family
One declaration may be submitted for a family traveling together, provided it includes no more than 10 passengers (including babies and children).
Permit/Visa Travelers
If you are travelling with a permit or other entry authorization, you must still complete this declaration (ASDAC).
